We are located at the corner of Hwy 2 and Pine Lake Road.
Please arrive 30 minutes prior to your first visit in order to complete the intake forms.
Please remember to bring with you:
- Current photo identification
- Insurance card(s)
- Specialist co-pay and/or co-insurance funding
- Any outside imaging reports/discs (this is not required if images were taken at Advanced Medical Imaging, Bryan Health Systems, or St. Elizabeth's Hospital)
- Current medication bottles or medication list from your pharmacy
